2) Enter Individual/Family Information
3) Review and Confirm Information

	Enter a Password* (Minimum six characters)

	Re-type your Password*

	Enter your E-mail Address

	Enter a Password Hint *


	Choosing a Password Your Password must be at least six characters long and can contain letters and/or numbers.

	Choosing a Password Hint Choose a Password that you’ll be able to remember when we give you your “Password hint.” For example, you might choose your dog’s name as your Password and enter “my dog” in the “Password hint” field.
