and it is possible given the circumstances, call your physician. |

At the hospital or urgent care center you will
need to provide your insurance company's name and your identification/subscriber
number. This information is on your Identification Card. |
Remember: Before you leave the hospital/urgent
care center, you will probably have to pay an emergency room co-pay. Your
co-pay amount is typically listed on your Identification Card. If the
person with the emergency is admitted into the hospital, the emergency
co-pay may be waived and an inpatient co-pay or deductible may be required
instead.
To ensure payment for the emergency services received,
you must notify your insurance company within a pre-determined amount
of time. The benefit summary explains what your particular plan requires
and covers for emergencies, or call the toll-free number on your Identification
Card.
